Tomasz Radzinski is having a medical at Fulham ahead of his proposed move from Everton.
The two clubs have agreed a fee of £1.75m for the 30 year-old forward, who rejected a new three-year contract at Goodison Park earlier this summer. Radzinski underlined his intention to move from Merseyside by handing in a transfer request, criticising manager David Moyes and suggesting that Wayne Rooney would need to leave Everton to further his career.
Fulham have held a longstanding interest in Radzinski, who has scored 26 goals in 101 appearances since joining Everton from Anderlecht after a protracted transfer saga in 2001.
